Assessment Tool for Depression Please check any question to which your answer is 'Yes'. Are you feeling sad or empty most of the day, nearly every day? Have you lost interest in all, or almost all, activities that you used to enjoy? Have you experienced a significant change in weight or appetite? Are you having trouble sleeping or are you sleeping longer than usual? Have others noticed that you seem agitated, restless, or "out of it"? Are you feeling tired or that you dont have any energy nearly every day? Are you feeling worthless or guilty? Are you finding it hard to think, concentrate or make decisions nearly every day? Do you have any thoughts of death or suicide? If you answer "Yes" to this question please seek help immediately by calling your local crisis line, mental health center, or 1-800-SUICIDE (1-800-784-2433).
